Sports Medicine

The UMass Boston Sports Medicine staff provides prevention, evaluation, management and treatment of athletic injuries to UMB varsity athletes.  Our staff includes licensed and board certified athletic trainers and an orthopedic physician. We are committed to providing the best possible care for all of our student athletes.

Student-athletes are required to completely fill out the appropriate forms and email them to Ed Perkins, Head Athletic Trainer at ed.perkins@umb.edu.  In the subject line of the email please specify the sport you are participating in.

Incoming first year students and transfer students must complete:

All new athletes and transfer students need to have a physical before participating in UMass Boston Athletics programs.  Please print out the sports physical form and bring it to your appointment with your physician.  Once form is completely filled out please mail it to the address below or bring it to either AJ Rourke or Ed Perkins in the Sports Medicine room.  If you choose to have Health Services at UMass Boston perform your physical, you do not need to print out the form.

In addition to your physical, you will need to provide your immunization records as well.

The Sports Medicine team encourages you not to wait until the first day of practice to complete your physical as it takes time to get doctor's appointments
and to gather immunization records.

*Any student that does not have a physical on record with Sports Medicine will not be allowed to participate in any practices and/or games.
